**East Bay Community Action Program (EBCAP) is seeking a full time Intake Coordinator for our Health Center at 100 Bullocks Point Ave, Riverside, RI.** The Intake Coordinator serves as the first point of contact for patients accessing EBCAP Health Center services, both in-person and via telehealth. This position is responsible for comprehensive intake procedures including patient registration, screening, assessment, and coordination with the healthcare team to ensure patients receive appropriate and timely care. ### Key Responsibilities - Work with health center clients and perform various screenings to determine client needs with respect to healthcare and/or social determinants. - Refer to appropriate services internally and externally as needed. - Provide specific assessments to Health Center clients to determine individual client needs.
