**Job Opportunity: Supports Coordinator – Intellectual and Developmental Disability Program** **Location: 1100 S. Additional starting salary incentives available for Master’s degrees and bilingual/ASL skills. - **Student Loan Forgiveness:** Qualify for student loan forgiveness under the Public Service Loan Forgiveness Program (PSLF). - **Supportive Team Environment:** Join a mission-driven team committed to exceptional collaboration and support. **What You'll Do:** As a Supports Coordinator, you will: - Facilitate the creation of person-centered support plans with individuals, families, and support teams. - Identify and refer community and system supports and services to meet individual needs. - Coordinate and monitor the delivery of supports and services as outlined in support plans. - Ensure the quality, type, duration, and frequency of services meet individual needs. - Monitor health and wellbeing of participants through regular contact with individuals, families, and service providers. - Identify unmet needs and document them for future planning and funding opportunities. - Inform individuals of their rights and maintain comprehensive documentation through a statewide web-based system. - Participate in ongoing training to enhance your skills and knowledge. **Qualifications:** - Bachelor’s degree with at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or related social science; OR - Associate degree with at least 12 college credits in the above fields and two years of relevant experience. - Required clearances: PA Child Abuse, PA Criminal History, and FBI. - Valid driver’s license and reliable vehicle. **Application Process:** If you are ready to join a dedicated team and make a significant impact in the lives of others, apply today!
