## **Employment Type:** Part time ## **Shift:** Rotating Shift # **Description:** Per diem Social Worker - provides psychosocial assessments, crisis intervention, discharge planning, and care coordination for patients across multiple hospital settings, including the Emergency Department, Medical/Surgical units, Birth Center, Intensive Care Unit (ICU), and Psychiatric Medical Unit (PMU).
