**About This Hospice Response Team Licensed Practical Nurse (LPN) Position** HopeHealth is seeking a Hospice Field LPN for its affiliate HopeHealth Hospice & Palliative Care. The Response Team LPN will respond to urgent patient needs and require excellent clinical assessment skills and an ability to quickly identify the best course of action to treat patients. This is not a Case Management position and requires an LPN who is comfortable in an action-oriented role who is comfortable assessing and treating a patient, then moving on to the next need. This is a part time position with 32-hours per week. The hours are 11:00am-7:30pm with a rotating weekly schedule. Week 1 Monday, Tuesday, Wednesday and Saturday. Week 2 Sunday, Tuesday, Wednesday and Friday and every-other weekend rotation, with (1) major and (1) minor holiday. **What will you do as a Hospice Response Team Licensed Practical Nurse (LPN) ?** - Initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial and environmental needs and activities of daily living. - Implements the individualized plan of care and recommending revisions to the plan as necessary and plans for discharge/transfer as appropriate. - Consults with and educates the patient/family/caregiver regarding the disease process and teaches the patient/family/caregiver self-care techniques, as appropriate.
