We offer a supportive, growth-focused environment along with a comprehensive benefits package, including: - Medical, Dental, and Vision Insurance, plus Wellness Programs - On-The-Job Training, opportunities for advancement and increased compensation - Generous Paid Time Off and Paid Holidays - Tuition Reimbursement and Certification Assistance - Mileage Reimbursement - 401(k) with Company Match - Additional Voluntary Coverage - Great Total Rewards Package!! **Qualifications** - Active RN License required - Hospice nursing experience preferred - Experience with Homecare Homebase (HCHB) preferred - Strong assessment, communication, and care coordination skills **Schedule & Work-Life Balance** - Monday–Friday, 8:00 AM – 4:30 PM - On-call opportunities available with additional compensation - Some locations offer dedicated on-call teams, resulting in backup coverage only **Key Responsibilities:** - Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care. - Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB). - Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care. - Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes. - Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
