Frequency of patient/family contacts will be at the discretion of the RN receiving after-hours calls and requests. This RN may also be assigned regular patient care visits as assigned by the RN Case Manager or Team Director. The RN endeavors to utilize teaching, assessment, and intervention skills to provide comfort care and maximize the quality of life for patients and families. **Responsibilities:** - Provides on-call coverage as scheduled: - Monday through Thursday 4:30 pm to 8:00 am, - Friday 4:30 pm through Monday 8:00 am, or - 7 on 7 off: 4:30 pm to 8:00 am during the week and Friday 4:30 pm through Monday 8:00 am, starting Wednesday evening and ending the following Wednesday at 8:00 am. - On call hours may include holidays unless requested off with holiday float hours or PTO.; May be asked to work outside of typical scheduled shifts. - Assesses patient/family needs, aware of the physical, emotional, and spiritual aspects and gathers data on social, economic, and cultural factors which may influence health, well-being and quality of life at all stages of care from referral to post death. - Assists patients, family members and all contacts according to the general performance standards, demonstrating organizational values with all interactions. - Provides direct care to patients within the RN scope of practice and consistent with organizational, professional and industry standards. - Evaluates and performs ongoing assessments and revises written plan of care with Interdisciplinary collaboration as the needs and conditions of the patient/family changes. - Authorizes, coordinates, and supervises care, as prescribed in the Interdisciplinary Plan of Care, with all team members and contracted vendors in order to meet the needs of the patient. - Documents accurate and ongoing patient information, within the patient medical…
