The Care Team Hospice is seeking a full-time Registered Nurse Case Manager to provide skilled nursing care and coordinated end-of-life support to patients in their homes and facility settings while collaborating closely with the interdisciplinary team. **Schedule, Hours & Territory** • Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM • Crown Point, IN, and the surrounding communities within approximately a one-hour driving radius. • RN Case Managers may participate in an on-call rotation with additional pay • Some branches offer dedicated on-call staff, resulting in backup on-call coverage only **What We Offer** • Strong interdisciplinary team collaboration • Supportive clinical leadership and resources • Clear expectations and consistent workflows • Environment focused on quality, compliance, and patient-centered care **Benefits** • Competitive Total Care Benefit Package, compensation, and mileage reimbursement per company policy • 401(k) with employer match • Health Savings Account eligibility with employer contribution up to $1,000 annually • Employee Assistance Program, discounts on gym memberships and wellness programs • Tuition reimbursement • PTO begins accruing upon hire, with full-time team members earning up to 3 weeks of PTO during their first year of employment and increasing accruals with tenure, plus seven paid holidays. **Key Responsibilities** • Conduct comprehensive patient assessments and develop individualized plans of care • Provide skilled nursing care in accordance with hospice standards and physician orders • Coordinate care with physicians, caregivers, and interdisciplinary team members • Educate patients and families on symptom management, disease progression, and end-of-life care • Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase) • Participate in interdisciplinary team meetings and care planning • Support after-hours patient needs as part of an on-call rotation when applicable • Active Registered Nurse (RN) license • Hospice or home health experience preferred • Strong clinical assessment and care coordination skills • Ability to work independently in the field • Strong communication and documentation skills • Experience using HCHB (Homecare Homebase) preferred **Field Role Requirements** • Active driver’s license • Ability to pass a criminal background check • Ability to pass a Motor Vehicle Record (MVR) check • Current TB testing in accordance with agency policy • Current CPR certification as required for the role
