The Care Team Hospice is seeking a **part-time CNA** to provide personal care and comfort-focused support to hospice patients in their homes and facility settings while working alongside our interdisciplinary team to ensure dignity, compassion, and quality of life throughout the hospice journey. **Schedule, Hours & Territory** • Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM • Ingham County, MI **What We Offer** • Opportunity to make a direct impact on patient comfort • Supportive clinical and leadership team • Mission-driven hospice environment • Competitive compensation and mileage reimbursement per company policy **Key Responsibilities** • Provide personal care, hygiene, and comfort measures • Observe and report changes in patient condition • Support patients and families with compassion and respect • Document care provided accurately and timely in the EMR HCHB (Homecare Homebase) • Active CNA certification • Hospice aide experience preferred • Experience using HCHB (Homecare Homebase) preferred **Field Role Requirements** • Active driver’s license • Ability to pass a criminal background check • Ability to pass a Motor Vehicle Record (MVR) check • Current TB testing in accordance with agency policy • Current CPR certification as required for the role
