Home Health Per Diem Physical Therapy Assistant (PTA)

Your Impactful Role as a Per Diem PTA - **Provide skilled physical therapy services** in patients' homes by implementing treatment plans established by the supervising Physical Therapist, helping patients improve mobility, strength, balance, and independence. - **Monitor patient progress and document care** accurately and promptly, communicating changes in condition and collaborating with the interdisciplinary care team to ensure high-quality patient outcomes. - **Educate patients** on home exercise programs, safe mobility techniques, fall prevention, and the proper use of assistive devices to support continued recovery between visits. **Qualification & Skills** - Must hold valid MN PTA license - Previous home health and/or outpatient experience strongly preferred - Able to travel around the Twin Cities Metro area to provide services to client's (we pay mileage!) - CPR/BLS Certification
