We are currently seeking an **Assessment Specialist** to join our **HOME Choice** program. As a **Assessment Specialist**, you’ll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. You’ll be part of a mission-driven team that values your expertise, supports your growth, and empowers you to make a real difference in your community. **Key Responsibilities** - Act as point-of-contact to assist the individual to live in his/her home and/or community. - Assist individuals served with linkages to service providers and community resources as needed. - Identify barriers to compliance with individual’s healthcare regime, appointments and screenings. - Work with individuals to create solutions to identified barriers and empower individuals to manage physician, and/or other key medical professionals, contacts, appointments and/or screenings. - Communicate and collaborate with the individual’s healthcare team, including, but not limited to, the facility Social Worker and Waiver Case Manager. - Adheres to the CareStar Rule in performance of job responsibilities. - Understands and complies with CareStar Policies and Procedures. - Maintains confidentiality as related to patient information.
