Home and Community Service Provider ***Villa of Hope helps youth and families rebuild relationships, recover from trauma and renew Hope for their fu*** ***ture.*** **JOB TITLE:** Home and Community Service Provider **(CFTSS CPST Provider)** **POSITION GRADE:** 79 **DEPARTMENT/PROGRAM:** Behavior Health Services **SUPERVISOR:** Behavioral Health Clinical Supervisor **ESSENTIAL FUNCTIONS:** - Provide rehabilitative psychoeducation, strength based service planning, and other restorative supports to children and adolescents up to age 21 within the community. - Exercises full compliance with the Agency’s Code of Conduct, all Agency policies and procedures and Federal and State laws and regulations - Maintains confidentiality and security for all client and staff related materials and/or records. - Provides crisis/emergency contact as needed, including being included in the on call rotation - Assesses patient needs and develops individualized treatment methods - Communicates closely with MCO’s, intake staff, finance and administrative assistants - Provides individual and group supervision to patients in care and accounts for patients at all times - Provides a safe environment for patients and family utilizing the Sanctuary Model - Reads and becomes familiar with program policy manual, including billing - Keeps current with the program training requirements for all program services - Employs approved crisis intervention/prevention techniques utilizing accepted practices per Agency policy and procedures - Documents critical incidents as required by program/agency - Assists with projects associated with the Program, including internal and external committees, task forces and special initiatives - Exercises daily independent decision-making, problem-solving and ongoing task prioritization - Participate in team meetings - Reviews the updates of Service Plans and/or like documents - Participates in group and individual planning process - Documents progress notes and other related documents as assigned in the Electronic Client Record within the day of service - Track and monitor progress, outcomes and utilization of each patient - Participates in surveys conducted, both internal and external - Demonstrates, promotes and practices cultural competency toward youth and staff through respect and understanding achieved through training and Agency cultural activities - Recognizes and embraces the diversity of teams - Supports and develops the strengths of each individual team member - Implementing and monitoring Individualized Service Plans - Participates in bi-weekly supervision and ongoing coaching sessions with Supervisor - Attends trainings as required and as appropriate to skills related to the position - Updates schedule weekly to maintain required expectations both fiscally and programmatically - Submit ADP and Company Miles, entries on a bi-weekly basis. - Coordinate with Supervisor for individual and group assignments - Communicate regularly with referral sources and patient’s team - Cooperates and collaborates with program staff, volunteers and other agency staff; both internal and external to the agency - Manage and update patient chart following the workflow and regulation requirements - Check and respond to e-mail and phone calls within 24 hours - Works to support program expansion by participating in services related to growth - Other similar duties as requested requiring comparable skills. **** **POSITION SPECIFICS:** **** **Education:** - A Bachelor’s degree and two years applicable experience in children’s mental health, addiction, foster care/child welfare/juvenile justice, and/or a related human services field and no certification in an Evidenced Based Practice **Experience:** - 2 years’ experience working with individuals and families in mental health, chemical dependency and/or human service settings - Valid NYS Driver’s License in good standing - Reliable automobile (travel is required with personal vehicle) - Proficient w…
