Transportation Driver # **Job Description** | **Department** | Transportation | | --- | --- | | **Reports to** | Administrator or Transportation Coordinator | | **Reporting to this position** | N/A | | **Job Classification** | Permanent Full-Time, Permanent Part-Time, Temporary on-Call | | **Position Purpose** | To provide reliable and safe Resident transportation to and from appointments. | **Required Qualifications** 1. Company drivers must be at least eighteen (18) years of age. 2. Current valid state issued driver’s license. 3. Maintain acceptable driving record 4. Be able to lift 50 pounds. 5. CPR and First Aid Certified (or willing to attain upon hire) 6. Exhibit excellent interpersonal and customer services skills 7. Ability to bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position. 8. Ability to work beyond normal working hours and on weekends and holidays when necessary. **Major Duties and Responsibilities** 1. Transport residents in a safe and timely manner to/from the appointments. 2. Provide residents with friendly, compassionate, reliable transportation services 3. Drive a large van in a safe, courteous, and reliable manner. 4. Gather and assist residents into wheelchairs or stretchers, in and out of the van, and other areas needed. 5. Daily cleaning and regular maintenance of vehicles 6. Coordinate schedules and appointments with the in-office Transportation team 7. Daily safety checks of the vehicle – ie: lights, brakes, tires, wheelchair lift, seatbelts, etc. 8. Secure all necessary paperwork for appointments and deliver it to appropriate people. **Additional Assigned Tasks** 1. Assist Nursing Staff as needed for transportation or transportation related activities. 2. Treats all residents with dignity and respect. Promotes and protects all residents’ rights. 3. Establishes a culture of compliance by adhering to all facility policies and procedures and complete all assigned training and education upon hire and routinely throughout employment. 4. Comply with standards of business conduct and state/federal regulations and guidelines. 5. Always follow appropriate safety and hygiene measures to protect residents and themselves. 6. Maintains confidentiality of protected health information, including verbal, written, and electronic communications. 7. Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel. Reports any retaliation or discrimination to HR or compliance officer. 8. Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor/Administrator/Abuse Coordinator. Protects residents from abuse and cooperates with all investigations. 9. Reports any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy. 10. Participates in all life safety and emergency drills and training. 11. Reports work-related injuries and illnesses immediately to supervisor. 12. Follows established infection control policies and procedures. 13. As a condition of employment, completes all assigned training and skills competency. 1. Meets general health requirements according to policy, including medical and physical exams and checking immunity status to various infectious diseases. 6.
