Our team is committed to partnering with our seniors to remain at home through promoting open communication and shared decision making while providing excellent care and services. **Schedule** - Monday - Friday, 8a - 430p - No Weekends - No Evenings - No Holidays - No On Call **Benefits** - Competitive Pay - Comprehensive Benefits - 401k - PTO - Paid Holidays Off **Requirements of the Financial Eligibility and Reimbursement Coordinator** - Bachelor’s degree with applicable experience in Health related Field (Preferred) - Must have a driver's license, reliable transportation, and car insurance - Knowledge of Medicare/Medicaid regulations (Required) **Responsibilities:** - Responsible for coordination of Medicaid (MA) and Medicare benefits with participants, families, and governing agencies. - Maintain current electronic case management records, including annual MA recertifications, Skilled Nursing Facility day 31 submissions, and invoicing. - Complete documentation of all visits (including episodic, routine, phone calls, home visits, etc.) against the corresponding care plan elements. - Communicate participant changes to team members. - Provide administrative assistance (applications, forms etc.)/community resources to participants as needed. - Assist participants in successful completion of the medical assistance recertification application including submission of all relevant documentation within required timeframes to maintain financial eligibility for Senior LIFE. - Keep the participant and family updated about the participant’s MA eligibility, Medicare application stats, and any changes with the participant’s financial status.
