Family Support Provider

## Description Shift: Thurs, Fri, Sat, E/O Wed. 7P-7A High School Diploma or equivalent. Experience in seeking and accessing state and community resources and a willingness to help families access resources. Experience raising a child with behavioral/emotional issues, accessing services for children with behavioral/emotional issues and navigating the child serving system. - Assist families in developing and recruiting informal supports to be a part of the Family Team meetings. - Complete (or co-complete) Strengths Needs Cultural Discovery (SNCD) assessments, Functional Assessments/Crisis Plans, Safety Plans, and Care Plans/Updates. - Provide support for families with the eventual goal of the family utilizing their natural supports. - Facilitate or co-facilitate Family Team meetings and ensure informal supports are included and part of the team. - Implement the Do For, Do With, and Cheer On model for families. - Maintain close communication with the Integrated Team Manager and other clinical and administrative staff in order to ensure the highest quality services are delivered for youth and families. - Complete flex-fund requests when necessary. - Monitor and evaluate services provided to the families. Contact families as needed to monitor progress and inquire about new needs. - Personality and mature presentation of oneself, demonstrating the ability to work with various systems while maintaining objectivity and cooperative attitude.
