FC Consultants/NP Practitioners meet with families in their homes to enhance their parenting capacities, utilize change-focused strategies, assist with family bonding and nurturing activities, and explore individual family needs to assist in providing support for long-term change. **JOB SUMMARY:** Family Connection Consultants will implement Family Connections (FC) model and curriculum with fidelity and follow all program and agency policies and procedures. This position has the potential to complete some position requirements from a home setting, client visits are conducted in the client home. **ESSENTIAL DUTIES:** 1. Implementation of the program in accordance with contract requirements, completion of Family Connections core training and DCS Standards of Practice. 2. Family Connection Consultant will conduct home visits and maintain case load as assigned. 3. Complete individual home visits at a minimum once a week and deliver FC Curriculum. 4. Prepare for home visits and complete documentation of visit within (1) business day of completion of the visit. 5. Conduct home visits at time that is convenient for the family. This may include evenings and weekends. 6. Conducts comprehensive family assessment and other assessments as required with families that accept services according to the Family Connections Model and contract guidelines. Works with DCS Specialist to engage and assess the family. 7. With participation of the family, develops and implements a Service Plan ensuring UBSMART goals, and the interventions that will support family members to achieve outcomes and goals. 8. Maintain case records and data system in accordance with agency and program policies, procedures, and requirements. 9. Initiate contact and conduct a home visit with referred families within one (1) business day of referral and actively work to engage families in the program. 10. Attend mandated virtual or in person trainings, conferences, meetings or community events, which may include out-of-state travel. 11. Connect families to resources that address their needs. 12. Maintain a positive working relationship with colleagues, administration, collaborative partners and others within the community. 13. Participate in a one-hour individual supervision and one hour group supervision per week. 14. Makes appropriate referrals to government & community agencies as needed. Make appropriate referrals to sub-contractors within the program using established procedures & forms. Follows up with sub-contractors regarding progress. 15. Report suspected child abuse or neglect to the Department of Child Safety (DCS). 16. Complete training in the Nurturing Parenting Program practice model. 17. Utilize or conduct intakes per the Nurturing Parenting model. 18. Collaborate with the parent/caregiver to gather information and complete the Family Nurturing Plan (FNP) that identifies the parenting constructs and competencies that will be the focus of NPP services. 1. Conducts a minimum of two hour weekly in person parenting sessions with family and documents family interaction and progress in a timely manner. 1. Schedule a mid-point session with the family, to occur halfway through the service request period, to evaluate the parent/caregiver’s progress towards achieving the NNP goals. 2. Close case when: a) targeted UBSMART goals have been addressed; b) the client requests case closure; c) provision of service can no longer achieve goals; or d) when identified reason(s) have been met per the Nurturing Program Manual.
