Your expertise will contribute to our commitment to delivering personalized and effective therapy programs. **Duties & Responsibilities** - Conduct comprehensive assessments to determine patients' functional abilities, limitations, and goals. - Develop personalized treatment plans based on assessments and goals, utilizing various therapeutic techniques and modalities. - Implement and administer therapeutic exercises, manual therapy, and other interventions to improve patients' mobility, strength, flexibility, and function based on each individual patient’s need. - Maintain accurate and organized documentation of patient progress and treatment outcomes. - Monitor and document patients' progress, adjusting treatment plans as necessary to achieve optimal outcomes. - Educate patients and their families about therapy procedures, home exercises, and injury prevention techniques. - Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive patient care. - Maintain accurate and timely documentation of patient evaluations, progress notes, and discharge summaries. - Stay updated on advancements in physical therapy techniques, technologies, and best practices, integrating them into clinical practice as appropriate. - Meet the needs of having patients execute Plan of Care and (but not limited) following up with patients if noncompliance occurs. - Be a team player to utilize nonclinical staff to support needs of the caseload. - Delegate and use a Physical Therapy Assistant (PTA) when applicable. - Participate in helping to drive a collaborative culture based on our service vision. - Adapt to change in patient schedules and daily patterns. - Adhere to ethical and professional standards, ensuring compliance with regulatory requirements and company policies. - Perform comprehensive patient assessments to determine therapy needs and establish treatment goals. **Skills & Qualifications** - Valid state licensure as a Physical Therapist - Doctorate or Master's degree in Physical Therapy from an accredited program - CPR certification and any other relevant certifications as required by state regulations - Strong clinical skills with experience in orthopedic, neurological, or geriatric physical therapy (preferred) - Excellent communication and interpersonal skills. - Ability to work effectively both independently and collaboratively within a team environment - Proficient computer skills for documentation and record-keeping purposes **Benefits & Perks** We offer…
