If you're passionate about making a difference and want to be part of a mission-driven organization rooted in faith, hope, and service, we invite you to join us in changing lives and strengthening our communities. **What You’ll Do:** **** - Provide a wide scope of activities for Developmental Disabilities Support Coordination services for a two-county area. - Work closely with program staff to provide initial and ongoing training. - Audits the accuracy and timeliness of ISPs, UR requests, case notes, budget, and other documents. - Attends interdisciplinary team meetings to discuss client cases in relation to eligibility, support plans, progress, and possible changes in the service program - Conduct pre-survey reviews to identify and correct deficiencies before SMH/CMS/MMAC reviews. - Assist Program Managers in gathering data for reviews. - Develop and maintain flowcharts and spreadsheets, tracking data and reporting findings to the team. - As needed, manage a selected caseload of clients with intellectual disabilities/developmental disabilities in placement or natural homes. - Performs other related duties as assigned.
