Day Program Specialist

**Position Title:** Day Program Specialist **Organization:** Hinds' Feet Farm ## **Position Summary** The Hinds' Feet Farm Day Program Specialist acts as an agent of Hinds' Feet Farm and is responsible for supporting members in accordance with their Person-Centered Plans, while ensuring safety, engagement, and overall well-being. * * * ## **Essential Duties and Responsibilities** - Must understand and accept that the needs of the member define the parameters of all staff duties. The Supervisor will outline any changes in staff duties as appropriate for the needs of the member. - Always consult with the Supervisor in the event of questions regarding a member's Person-Centered Plan. - Cooperate with and work in tandem with the Supervisor to facilitate member engagement in the Day Program. - Participate in training provided to learn about brain injury, best practices, individual member needs and goals, correct documentation, and other job-related responsibilities. - Work with members to address goals outlined in Person-Centered Plans, including but not limited to: - Basic ADLs - Socialization - Personal hygiene and grooming - Health support - Personal assistance - Feeding, cooking, laundry, and cleaning - Transfers - Handling of bodily fluids (using universal safety precautions) - Recreational, vocational, and educational activities - Nutrition - Emotional support and hobbies - Educate members, families, and guardians on member rights and responsibilities. - Note all changes in member diagnosis or medication and communicate with the Supervisor. - Collaborate with the member's guardian and/or program staff to maintain open communication regarding the member's progress. - Provide guidance, support, training, and education to members, families, and guardians. - Maintain open, clear communication between members, families, and providers. - Provide necessary supervision of members in all aspects of the program when needed; members must never be left alone. Staff must remain awake and alert at all times (e.g., not using a cell phone inappropriately). - Document member progress appropriately and submit in a timely manner; ensure compliance with the NC DHHS Records Management and Documentation Manual and Hinds' Feet Farm's Policies and Procedures. - Report all incidents and ensure member safety in accordance with the NC Division of MH/DD/SAS Incident Response and Reporting Manual. - Follow all applicable rules set forth in: - 10A NCAC 27 Subchapters C, D, E, F, and G - 10A NCAC 26B Confidentiality Rules - Transport members to and from the program and within the community. - Plan and facilitate group activities. - Participate in Hinds' Feet Farm staff meetings as requested by the Supervisor. - May be asked to work one-on-one with members who have higher behavioral management needs. - Perform other related duties as assigned. * * * ## **Skills and Qualifications** - Must have no substantiated findings of abuse or neglect and be in good standing with the North Carolina Health Care Personnel Registry. - High School Diploma or equivalent required. Must be at least 21 years of age. - At least one year of experience preferred in teaching or working with individuals with developmental disabilities and/or mental health needs. - Valid driver's license with a minimum of four years of driving experience, current car insurance, and a clean driving record; willingness to drive a 12-passenger van required. - Must consent to a pre-employment drug screen (to be completed within 48 hours of hire).
