(FT/PT) The Connections Care Coordinator is responsible for maintaining and monitoring the memory care (Connections) holistic wellness program, including but not restricted to: assisting the Director of Engagement with input into resident assessments, leading neuroplasticity programming (Connect-Move-Learn cycles),, muscular strength, aerobic and flexibility training, wellness promotion, social-recreational activities and outings and volunteer program development and community resource coordination. Highlights of the Connections Care Coordinator Position: High school diploma or equivalent Minimum 1 year experience activities or recreation service experience Certified Dementia Practitioner or eligible for certification. Willingness to work in a hands-on role with residents and staff.
