Achieva provides personalized services and opportunities that respect each person's unique aspirations. **Position Summary** The Complex Care Specialist is responsible for supporting people with intellectual and developmental disabilities. This position works with assigned staff and independent contractors to support personal outcomes and ensures compliance with regulatory, internal, and external policies. The specialist assists in charting a course that supports each person in realizing their life of personal significance, in collaboration with their family and other team members. **Key Responsibilities** - Coordinates and contributes to the development of plans mandated by regulations, ensuring implementation and adherence to each person's Person-Centered Plan and current Individual Support Plan (ISP). - Revises the Individual Support Plan to reflect the goals and outcomes chosen by the person supported. - Ensures the accuracy of ISP contents and coordinates the completion of assessments as requested by external partners. - Respects each person's right to make choices and exercise self-determination, supporting their health, safety, and right to risk. - Ensures opportunities are provided for each person to explore their interests in the community and form relationships. - Maintains ongoing communication with all necessary people to appropriately support each person. - Performs miscellaneous duties as requested and/or required. - Associate Degree from an accredited college or university and four (4) years of work experience working directly with people with an intellectual disability. - Bachelor's Degree from an accredited college or university and two (2) years of work experience working directly with people with an intellectual disability. - Master's Degree from an accredited college or university and one (1) year of work experience working directly with people with an intellectual disability. - Successful completion of Medication Administration training, provided by Achieva. - Career Development and Advancement Opportunities, in partnership with the National Alliance of Direct Support Professionals (NADSP). Acquire Front Line Supervisor (FLS) badge/certification within one (1) year of enrollment, with corresponding pay rate increase and bonus incentive. - Completion of Mental Health First Aid, Trauma-Informed Care training, and Frontline Supervisor training within the first year of employment. - Satisfactory Act 33/34 clearances and FBI background check. - Valid Pennsylvania driver's license and properly insured vehicle (mileage reimbursement provided for travel using a personal vehicle). - Must obtain First Aid/CPR certification within two (2) months of the start date. - Functional Behavior Assessment (FBA) Certification required within one (1) year of em…
