The Community Services Coordinator supports the mission of the Adams County Collaborative Management Program (ACCMP) to assist children, youth, and families in Adams County. This role advances department objectives to Elevate People, Improve Systems, and Strengthen Neighborhoods. The position focuses on prevention, intervention, and improving access to community resources through the Partnering for Adams County Kids (PACK) program, using a Two-Generation (2Gen) approach for families with children from birth to age 21.**PACK Meeting Facilitation and Coordination** - Coordinate and facilitate family-centered PACK meetings using a 2Gen approach. - Manage meeting logistics including scheduling, communication, materials, follow-up, and action planning. - Maintain accurate and timely documentation, including meeting notes, case information, referrals, and data entry. - Build trusting relationships with families to identify strengths, needs, goals, and priorities. - Support families in accessing and navigating community resources and public systems. - Collaborate with departments, schools, nonprofits, and government agencies to strengthen coordination of community-based support. **Community Engagement and Outreach** - Serve as a point of contact for families, caregivers, and community partners to assess needs, identify strengths, and connect to resources. - Attend community meetings, outreach events, and collaborative gatherings to strengthen partnerships and community trust. - Facilitate conversations with residents and stakeholders to identify service gaps, barriers to accessing services, and opportunities for improvement. - Represent ACCMP and CSWB initiatives on boards, committees, workgroups, and collaborative efforts. - Participate in outreach activities that increase awareness of available programs and resources. **Administrative and Program Support** - Maintain organized records, databases, and documentation related to program activities, referrals, outcomes, and community engagement. - Assist with data collection, analysis, and reporting to inform program development and implementation. - Partner with internal data and evaluation teams to support data-informed decision making. **Required Knowledge, Skills, and Abilities** - Strong interpersonal and relationship-building skills with the ability to engage effectively across various cultural and community contexts. - Knowledge of community resources, public systems, nonprofits, local government, and cross-sector partnerships, particularly those serving residents in Adams County. - Skilled in facilitating collaborative meetings and navigating complex family and system dynamics with professionalism and compassion. - Strong organizational skills with the ability to manage multiple priorities, projects, and deadlines. - Ability to develop and maintain productive working relationships with community members, partners, and colleagues. - Bilingual (Spanish/English). - Demonstrates strong technology proficiency, including use of spreadsheets and data management tools. - Produces clear, concise, and professional written communication, including reports, summaries, and program documentation. - Ensures accuracy and attention to detail in data collection, entry, analysis, and reporting. - Apply foundational evaluation skills to support program assessment and measure effectiveness. - Contributes to process improvement efforts by identifying inefficiencies and recommending practical solutions. **Preferred Skills** - Experience working within collaborative management models, multi-agency teams, or collective impact initiatives. - Familiarity with child welfare, behavioral health, housing, or human services systems in Colorado. - Experience using project management tools. - Proficiency with data visualization tool…
