Community Nurse Liaison LPN

**American Senior Communities is now** **hiring a** **Community Nurse Liaison** **(LPN)** **Hours: Saturday and Sunday 8a - 5p** **Remote position but must live within reasonable driving distance to Indianapolis** The **Clinical Nurse Liaison** provides patient evaluations, while collaborating with hospital personnel to determine patient’s clinical needs and appropriateness for admission to skilled nursing facility. This position works closely with our communities and healthcare partners.
