Learn more at www.sabathani.org. **About the role** Sabathani Community Center's Health Equity Department works to reduce health disparities and improve access to care for community members through direct service, care coordination, and community education. The department's programming is designed to meet residents where they are, with a focus on culturally responsive, trauma-informed care. Sabathani Community Center is seeking two Community Health Navigators to support the Live Well at Home program, a comprehensive, culturally responsive initiative that helps older adults (65+) safely age in place and reduce the risk of nursing home placement. The Community Health Navigator connects seniors to health, wellness, and preventive programming; conducts on-site health screenings and follow-up navigation; coordinates services and referrals through Sabathani's Senior HUB; and supports transportation access and social engagement programming. This role is well suited to someone who enjoys direct relationship-building with older adults, is comfortable coordinating across programs and partner agencies, and is committed to ensuring that elders in South Minneapolis feel seen, celebrated, and connected. The physical duties associated with this position are consistent with duties associated with work performed in an office and community setting. To perform this job successfully, you must be able to carry out each essential job function satisfactorily. **What you'll do** Health Screenings, Education & Navigation • Conduct quarterly on-site health screenings (blood pressure, diabetes, vision, balance, and other preventive screenings) in partnership with Southside Community Health Services and other healthcare providers, following completion of required screening training. • Provide follow-up navigation and referrals based on screening results to ensure participants are connected to primary care, behavioral health, and ongoing services. • Support health education workshops and senior-focused conferences on chronic disease prevention and management, aging in place, caregiving, and health equity. • Provide digital health literacy support, including patient portal (MyChart) navigation, telehealth access, appointment scheduling, and basic computer skills. Service Coordination & Senior HUB • Serve as a primary point of contact within the Senior HUB, a centralized "front door" connecting seniors to services and referrals. • Provide benefits navigation, including insurance, financial resources, and tax support, and coordinate access to legal services such as estate planning and advance directives. • Develop individualized support plans that address risks related to isolation, safety, and daily living needs. • Coordinate service navigation to connect seniors to home and community-based services, including homemaker, chore, companion, and respite supports. • Use warm handoff procedures with partners, including Volunteers of America and Southside Community Health Services, to reduce duplication and delays in service.
