CNAs must exhibit competencies required by law and abide by Four Seasons policies in providing personal care to patients, as directed by the written plan of care. ## **** ## **Essential Duties & Responsibilities:** - Practice within the scope of license, certification or registration, where applicable, and as required by law and regulations as delegated to and directed by the registered nurse: - Participates in the coordination of the patient’s hospice care in accordance with professional standards of practice - Collects data for ongoing interdisciplinary comprehensive assessments - Participates in the development and evaluation of the plan of care by reporting observations - Contributes to patient and family counseling and education - Perform duties in compliance with Medicare Condition of Participation - Practice safety and infection control methods - Provision of hands-on personal care - Works to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies ## **** ## **Knowledge, Skills & Abilities:** - Must be willing and able to care for patients with infectious disease processes unless approved accommodations - Knowledge of proper body mechanics - Excellent organization skills - Excellent interpersonal skills - Excellent written and oral communication skills - Ability to work under pressure to meet deadlines - Ability to take initiative and utilize innovative techniques and ingenuity - Ability to interact with various departments throughout the organization - Ability to work flexible hours, if needed - Ability to participate as a team player to coordinate projects - Ability to read, interpret and apply laws, rules and regulations - Ability to plan, prioritize and coordinate multiple projects ## **** ## **** ## **Minimum Qualifications:** **** ***Education:*** - High School Diploma/GED ***Experience:*** - No more than a 24 month lapse in providing clinical care services upon the completion of a State-Accredited Certified Nursing Assistant course - 0 – 1 year of clinical experience. ***Licensures & Certification…
