Participate in continuous quality improvement activities 10. Assist with outreach activities such as health fairs, when needed 11. Document client encounters from outreach events, activities, service plans, and outcomes achieved by client in an effective manner; completes and submits monthly reports; and ensures accurate data collection for quality measures and other reporting purpose 12. Coach patients in effective management of their chronic health conditions and self-care. Assist patient in understanding care plans and instructions. Motivate patients/clients to be active and engage event participants in their health and overall wellbeing. 13. Assist patients in accessing health-related services to appropriate medical and specialty providers and overcoming barriers to obtaining needed medical care and /or social service 14. Comply with Health West Inc.s policies and procedures 15. Other duties as assigned ***Education*** High School Diploma or Equivalent Graduation from a Medical Assistant Program or Equivalent Graduation from a Practical Nursing Program or Equivalent ***Experience*** One year recent experience in a health care setting preferred. Current National Medical Assistant Certification required. Current Idaho Practical Nursing License or equivalent required. Current certification in BLS.
