Clinical Case Manager - Health Professional III

- ## Department Information - This position is open to current Colorado residents only. Located within the Colorado Department of Human Services (CDHS), the Civil and Forensic Mental Health (CFMH) is comprised of the two state inpatient acute-care psychiatric hospitals, Forensic Services, Mental Health Transitional Living Homes, Adolescent Psychiatric Residential Treatment Facility (PRTF), as well as all of the critical units that enable the work of the hospitals and other Divisions, which include: financial services, information management, Medicare/Medicaid and HIPAA compliance, hospital safety and security, and the CMHIP police. The program this position works within Outpatient Services and will support individuals in the program who will be transferred from the department’s designated treatment locations from county jails in the State of Colorado, as well as from the Colorado Mental Health Institute in Pueblo (CMHHIP), the Colorado Mental Health Institute in Ft. Logan (CMHHIFL), to private hospital locations for inpatient Civil Commitment. - ## Description of Job - This position is remote. The work location will be your home address, with on-location work and travel for in-person meetings primarily in the Denver-metro area. Please Note: These working arrangements are subject to change. **What You'll Be Doing:** - Will administer appropriate/evidence-based assessment instruments to determine the individual needs of clients with consideration to key variables such as education level, language, and culture. - Must interpret and apply the needs assessment results, incorporating collateral information, to identify the required complementary treatment or social services. - In collaboration with the Care Coordination Program and other necessary stakeholders, the case manager will develop case plans designed to ensure client coordination of care and placement acquisition. - Through the established case plan, the case manager will provide and/or refer individuals for indicated services by facilitating, obtaining, and executing all necessary documentation related to care needs and placement. - This position coordinates access to identified needs within various systems, such as medication evaluation and management, mental health treatment, substance abuse treatment, case management, and other treatment services related to the client’s behavioral health needs and related court orders. - The case manager must remain in close contact with the Care Coordination Program, jails, hospitals, courts, and other relevant stakeholders to ensure continuity of care and clear communication regarding cross-agency clients. - This position serves as an intermediary between the healthcare and behavioral health systems, social assistance programs, county jails, courts, and the community, facilitating access to services in support of appropriate placements and community transitions. - It is expected that this position remains well-informed about various service options throughout Colorado, the associated processes, and the requirements. In addition, this position will establish and maintain strong working relationships with such stakeholders to support an understanding of our population's needs and positive outcomes. As needed, clinical case managers may need to transport individuals using State vehicles to engage with placement. - The Case Manager serves as the subject-matter expert regarding benefit options, processes, and requirements and may provide consultation on behalf of the Care Coordination Program in this domain. - This position is responsible for identifying clients in need of benefit acquisition, accepting referrals from the Care Coordination Program, and other stakeholders, and overseeing the benefit acquisition lifecycle.
