Are you a dedicated Registered Nurse leader with a passion for home health and patient-centered care? We are seeking an experienced Clinical Care Manager (RN) to lead a multidisciplinary team of clinicians in delivering exceptional home healthcare services. In this role, you will combine your clinical expertise, leadership skills, and commitment to quality outcomes to support both patients and care teams across the community. As a Clinical Care Manager, you will play a vital role in ensuring the delivery of high-quality skilled nursing services, supporting clinical staff development, maintaining regulatory compliance, and driving positive patient outcomes in a fast-paced, collaborative environment. What You'll Do Clinical Leadership & Team Management Lead, mentor, and support a high-performing team of Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and other clinical staff. Foster a collaborative, patient-focused culture built on teamwork, accountability, and compassion. Participate in onboarding and training of new clinicians, including shadowing programs, competency development, and OASIS education. Provide coaching, ongoing performance feedback, and professional development opportunities for team members. Partner with Clinical Directors to conduct performance evaluations and support employee growth. Patient Care Oversight Ensure the delivery of safe, high-quality, evidence-based patient care in accordance with physician orders and agency standards. Oversee patient assignments, care coordination activities, referrals, and care plan implementation. Monitor patient progress and ensure individualized plans of care are continually assessed and updated. Support case managers through regular caseload reviews and clinical guidance. Promote excellent patient satisfaction by proactively addressing concerns and resolving issues. Quality & Compliance Review daily clinical reports and on-call activity to ensure continuity of care and timely communication. Monitor clinical documentation, including OASIS assessments, visit notes, authorizations, and other required records for accuracy, completeness, and timeliness. Collaborate with operational and clinical leadership to achieve key performance indicators (KPIs) and quality metrics. Ensure compliance with state, federal, Medicare Conditions of Participation (CoPs), HIPAA requirements, and agency policies. Investigate and follow up on field incidents and patient care concerns as necessary. Clinical Supervision & Case Reviews Conduct comprehensive caseload reviews at least every 60 days and as needed. Evaluate care plans, payer requirements, clinical documentation, supervision needs, and patient-specific resources. Recommend care plan improvements and provide clinical education based on review findings. Conduct annual and as-needed field supervision visits to assess quality of care, patient safety, infection control practices, documentation compliance, and adherence to the plan of care. Collaboration & Communication Partner with physicians, caregivers, rehabilitation services, home health aides, and interdisciplinary team members to ensure coordinated care delivery. Participate in clinical team meetings, case conferences, referral partner meetings, and other collaborative initiatives. Support continuity of care through effective communication across all stakeholders. Qualifications Required Graduate of an accredited School of Nursing. Current, unrestricted Registered Nurse (RN) license in the state of practice. Successful completion of the NCLEX examination. One of the following: Bachelor's Degree in Nursing (BSN) and at least 1 year of related experience, or Associate Degree or Nursing Diploma and at least 2 years of related experience.
