## Description - Complete psychosocial assessments for residents to identify their mental, emotional, and social needs. - Meets with all new Residents to complete paperwork for medical record according to “Admissions to the Clinic” policy. - Provides individual and family counseling. - Supports Residents who no longer drive with guidance with arranging transportation. - Assists Residents in reviewing health insurance options during open enrollment periods. - Initiates and conducts support groups such as caregiver, bereavement, women/men groups. - Facilitates outside resources and services. - Coordinates companion program for Residents needing extra support at home. - Acts as a liaison with outside mental health practitioners. - Provides vocational assessment of Residents who are receiving Rehabilitation. - Documentation of progress notes and other social work entries to be recorded to permit and encourage regular and ongoing communication with physicians and other professionals involved in the resident’s care and billing. - Initiates advance care planning discussions with Residents including goals of care. - Visits new Residents in their home within a eight weeks after moving in to assess adjustment/transition and invites new Residents to attend “It Takes A Village”. - Follows Residents with cognitive impairment to assist with monitoring of their status and needs. - Promote facility–community interaction by encouraging community involvement, developing community resources, and participating in the assessment of and the need for planning related to other long-term social and health care resources. - Committed to working with residents and families to develop and implement strategies that promote person-centered care. - Applies “5 Star” customer service approach in all aspects of job. - Adheres to all company policies, procedures, and standards of conduct. - Reports to the appropriate managers any known or suspected violations of policy and procedures, regulations or standards of conduct. - Attends required in-services and training. - Performs additional duties as assigned. **Qualifications:** - Master’s degree in social work with 5 years of clinical experience in individual and family counseling. - Current LCSW licensure in the Commonwealth of Massachusetts.
