Department: Health & Recovery Services: Urban Response Job Category: Salary Reports To: Supervisor, Urban Response Position Type: Fulltime (1.0 FTE) Temporary - 8 Month Contract Direct Reports: N/A Shift: Day Shift Indirect Reports: N/A Location: Main building Position Summary Reporting to the Supervisor, Urban Response, the Licensed Practical Nurse primarily works in Urban Response programs, which include the Episodic Medical Care Clinic & Medication Assistance program. These programs support clients utilizing Emergency Shelter services that are in various stages of their housing and recovery journeys. The Episodic Medical Care Clinic (EMCC or “the Clinic”) program provides medical and social support(s) to clients experiencing homelessness and acute medical concerns, as well as supports the Medication Assistance program. This role is a part of the integrated Health & Recovery Services team and is expected to perform tasks related to medication management and teaching, applying various standardized assessment tools, symptom management, documentation, health promotion, referrals to internal and external resources, clinical interventions such as wound care, phlebotomy and specimen collection, and other tasks, as assigned by a Team Lead. The Licensed Practical Nurse will work closely with physicians and pharmacists to ensure that clients have all the resources required to initiate and maintain prescription medication treatments, such as income support, regular follow up with authorized prescribers, and routine check-ins to ensure medication regimes are appropriate for the clients’ care plan and/or goals. Additionally, the Licensed Practical Nurse will work with their multidisciplinary teammates to ensure that appropriate processes, such as the Intake, Goal Setting, Treatment Planning, and Discharge processes are followed, referred to with the client regularly, and clearly documented for other healthcare professionals to also support the clients’ goals. Time Commitment 40 hours per rotation with 4 days on and 3 days off. Some unusual hours may be required from time to time as the need arises. Hours are generally 0730-1730 hours, Tuesday to Friday however the schedule may be altered at the discretion of management. Job Duties and Responsibilities Perform delegated independent nursing functions within the scope of practice as determined by CLHA and the DI within the individual LPN competence level, as observed by the Urban Response Program Supervisor and/or other Health & Recovery Services Leadership team. Assist with other frontline staff duties, when time allows. Consistently demonstrate the ability to take the initiative to make decisions/choices without direct supervision. Be informed of APL routines and maintain standards and knowledge around phlebotomy and lab services. Participates in formal and informal health teaching including preventative health concepts concerning Hepatitis A, B, C and HIV, condom usage, nutrition, STBBI’s, basic sanitation, personal hygiene, post-op, and pre-natal care. Participate in influenza, other vaccination, and/or other specialist medical clinics from time to time. Maintains and ensures best practices around the handling of vaccines, participates in the maintenance of immunization procedures and administration of vaccines, medications, and other treatments necessary to provide client care. Ensure compliance with regulatory standards (SDS, OH&S, WHMIS). Organize job functions and work area to be able to effectively complete assignments within established time frames. Provide culturally aware support to DI clients, celebrating the diversity of clients and being aware of power dynamics of privilege and oppression that may be present. Ensure all required data is entered accurately into organizational databases including the DI Guestbook Database, and client Electronic Medical Records (EMR) and referrals provided in accordance to DI policy and professional nursing standards.
