## Description # Mental Health Children's Case Manager **Join Fernbrook Family Center and make a meaningful difference in the lives of children, adults, and families navigating mental health challenges.** Fernbrook Family Center is seeking a compassionate, organized, and self-motivated **Mental Health Case Manager** to provide Targeted Case Management services to individuals who meet Minnesota State Statute requirements for Serious and Persistent Mental Illness (SPMI) or Severe Emotional Disturbance (SED). We value diversity, inclusion, collaboration, and the unique lived experiences of our team members. ## Position Summary The Mental Health Case Manager works closely with children, adults, families, treatment providers, schools, community organizations, and other support systems to coordinate services and advocate for client needs. This position offers flexibility, autonomy, and the opportunity to build meaningful relationships while helping clients achieve their goals. ## What You'll Do - Provide mental health targeted case management services to children, adults, and families. - Assess client needs and develop collaborative plans to connect individuals with appropriate services and resources. - Coordinate referrals and collaborate with community providers and support systems. - Maintain timely, accurate, and compliant documentation. - Meet with clients regularly based on identified mental health needs (minimum once every 30 days). - Facilitate and coordinate quarterly team meetings with clients and support networks. - Support discharge planning and transitions from residential, inpatient, correctional, or treatment settings. - Advocate for clients and families to address barriers, navigate systems, and resolve crises. - Participate in supervision, team meetings, training opportunities, and ongoing professional development. - Complete required continuing education and maintain compliance with agency policies and state regulations. ## Qualifications Candidates must meet **Minnesota DHS Case Manager qualifications**, including one of the following: - Bachelor's degree in a behavioral science or related field (such as Social Work, Psychology, Nursing, or a related discipline) from an accredited college or university; **OR** - Three to four years of experience as a Case Manager Associate; **OR** - Registered Nurse without a bachelor's degree with specialized psychiatric training and three years of qualifying mental health experience; **OR** - Completion of 4,000 supervised hours providing mental health services; **OR** - Completion of 30 semester hours in behavioral sciences and 2,000 supervised hours providing mental health services. ### Additional Requirements - Valid driver's license, reliable transportation, automobile insurance, and acceptable driving record. - Ability to pass a criminal background check in accordance with agency and Minnesota Department of Human Services requirements. ## Preferred Qualifications - Experience working with children, adults, and families. - Previous case management experience. - Strong knowledge of community resources and support systems. - Experience assessing diverse client needs and coordinating services. - Bilingual or multilingual abilities. ## Skills for Success - Strong ethical judgment and professional boundaries. - Excellent organizational and time-management skills. - Effective written and verbal communication. - Ability to work independently while collaborating with multidisciplinary teams. - Openness to feedback, learning, and professional growth. - Basic computer proficiency and willingness to learn agency…
