**JOB SUMMARY** Collaborate with the Children and Families Counselor to coordinate services, resources, and support for children and adolescents whose guardians are participating in Hope Haven's residential programs. This role focuses on case management, service coordination, and resource linkage rather than direct clinical treatment. **Essential Duties and Responsibilities:** - Upon admission, engage children and adolescents and their families to build rapport and identify appropriate goals for the case management service plan - Coordinate and monitor children's and adolescents' access to individual and group counseling, therapy, and other clinical services provided by licensed clinical staff, as scheduled - Complete documentation requirements and maintain quality, up to date case management records, including client database - Facilitate referrals to appropriate community resources when needed - Provide information to referral sources and other authorized representatives as appropriate and within confidentiality guidelines - Coordinate and help facilitate parenting and family psychoeducational programming, in partnership with Children and Families Counselor - Participate in ongoing supervision and seek supervision when needed - Adhere to the Resident’s Rights Policy - Maintain professional and ethical boundaries. - Develop community networks with elementary and middle schools in Mecklenburg County. - Collaborate with local agencies, schools, and community organizations to achieve program objectives. - Offer direct support to families, including case management, family documentation, and referrals to resources. - Conduct needs assessments to identify and address the specific needs of families and children. - Coordinate and help facilitate workshops, training sessions, and support groups tailored for families, partnering with clinical staff as needed. - Implement initiatives that foster positive child development and enhance educational outcomes. - Advocate for the needs of children and families within the community, engaging with stakeholders to promote their interests. - Maintain detailed records of program activities, family interactions, and service outcomes. - Provide crisis support by connecting children and families to appropriate emergency and clinical resources and coordinating with the clinical team on intervention and follow-up planning. - Develop and implement crisis response plans, ensuring families are informed about emergency resources. **Education/Experience:** - Bachelor’s Degree in the Human Services field required. - Minimum of one (1) year of experience and/or specialized training working directly with children and/or adolescents and their families is required. - Knowledge of resources available in the community to support recovery. - Strong interpersonal and engagement skills - Strong organizational and time management skills - Excellent written and verbal communication skills - Knowledge of the SUD and its treatment. - Knowledge of recovery support systems and a commitment to the recovery principles. - Ability to perform duties physically and mentally without harm to self or others. - Must know confidentiality laws so that client confidentiality is always maintained. **Skills:** - Strong interpersonal and communication skills. - Proven ability to collaborate effectively with diverse populations and stakeholders. - Exceptional organizational and time-management skills. - Proficiency in Microsoft Office Suite and other relevant software. - Solid understanding of applicable local, state, and federal regulations and available resources. **Physical Requirements:** - Keyboarding - Walking/Sitting - Lifting
