## Description **COUNTY OF GREENE JOB DESCRIPTION** **POSITION:** County Caseworker 1 (2 Vacancies) Supports Coordinator-IDD UMWA Union Position **DEPARTMENT:** Human Services--IDD **LOCATION:** Fort Jackson Building **SALARY:** $20.08/hour **HOURS:** 35 Hours per Week (Starting and Ending to be set by Management) # OVERALL OBJECTIVE OF JOB: Employee ensures local access to a comprehensive array of quality case management services necessary to help individuals and their families lead more fulfilled lives in their communities. Greene County Human Services, IDD program will strive to effectively manage and remain responsive to a dynamic and changing health care environment. This professional casework position in the IDD Supports Coordination is responsible for the case management of IDD consumers. This position is primarily responsible for the locating, coordinating, and monitoring of services and referral to services, and the application of a consistent advocacy position for all consumers in IDD Services. # ESSENTIAL FUNCTION OF JOB: *\*\*\*\*MUST PERFORM ESSENTIAL FUNCTION S AT OR ABOVE THE DESIGNATED PRODUCTIVITY* *STANDARD\*\*\*\*\** 1. Schedule and facilitate Individual Service Plan Meetings and perform all necessary updates to individual’s ISPs in the Home and Community Services Information System (HCSIS) adhering to required timeframes. 2. Provide Face to Face case management services to all active consumers in accordance with Federal Regulation including monitoring to evaluate the quality and appropriateness of services and/or determine the need for additional or alternative service. Interpret the program and all applicable policies and procedures to individuals as needed. 3. Works in close cooperation with Supervisor, Provider Managers, other agencies, hospitals, clinics, courts, school districts, and other community resources in planning to meet the needs of IDD individuals. Attend regular review meetings with Providers and other social service agencies that work with the individuals. 4. Attends weekly departmental staffing and other regular meetings with Supervisory staff as assigned. 5. Prepares correspondence and reports as requested by the Supervisor including, but not limited to, Independent Monitoring for Quality reports. 6. Updates and maintains information required in department charts, HCSIS and the Agency’s Consumer Management Computer System in accordance with ODP regulations and Agency policy. Maintains accurate records and service notes applicable to billable & non-billable time according to ODP guidelines and agency set standards. 7. Participate in annual staff development trainings as required by County, the Office of Developmental Programming, or the Center for Medicare and Medicaid Services. # OTHER DUTIES OF THE JOB: 1. Attend training sessions as needed and as approved by supervisor 2. Act as a resource for individuals requesting information 3. Assist other workers in meeting the needs of the consumers 4. All other duties as assigned. 5. Travels to client homes, providers, meetings, trainings, and/or other related county business as required. # SUPERVISION RECEIVED: Receives regular instruction and ongoing supervision in regard to daily work duties from Supervisor. # SUPERVISON GIVEN: None. # WORKING CONDITIONS: 1. Works indoors and outdoors in adequate work space, lighting, ventilation and temperatures. 2. Works with above average exposure to stress and noise, but subject to frequent disruptions. 3. Normal indoor exposure to dust/dirt. 4. Works outdoors in all weather conditions. # PHYSICAL/MENTAL CONDITIONS 1. Must possess ability to record, convey and present information, explain procedures and follow instructions. 2. Must be able to sit for long periods throughout the workday, with intermittent periods of standing, walking, bending, twisting, reaching and driving as necessary to carry out essential job duties. 3.
