Case Manager II - Targeted Case Management (Dakota County) People Incorporated is seeking a **Case Manager II** to join our **Targeted Case Management Dakota County** team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. We connect people with services through developing person-centered support plans that help them live as independently as possible. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. **Hours:** Full - Time, Monday - Friday, business hours **Location:** Dakota County, Minnesota l Hybrid **Range:** $24.30 - $25.35 Hourly, based on qualifications **Hiring Bonus:** $1,000 to external candidates **People Incorporated Mental Health Services** is the largest nonprofit mental health provider in Minnesota. For over 50 years, we have been a leading community partner, transforming the health of our communities through innovative solutions. **Main Job Duties:** - **Must be able to have onsite presence within Dakota County, Minnesota** - Attending bi-annual meetings with clients to monitor services - Develop and maintain a collaborative relationship with clients serving as a role model for clients by demonstrating positive, proactive and respectful communications - Assessing client needs and ensuring continuity of care. This may include outreach and consultation with other partners and providers. - Maintain regular contact with collaborative partners and make referrals to professionals, schools and community agencies, as needed - Making referrals and authorizing services based on needs - Maintaining records and documenting all coordination activities - Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards ## To perform this job successfully an individual must be able to perform each essential duty satisfactorily.
