Position Summary: The Case Manager for the Encampment Resolution Funding (ERF) program provides outreach services to unsheltered individuals and households residing in San Mateo County’s Critical Homeless Encampment Zones (CHEZ). The primary focus is on CHEZ sites in San Mateo, Burlingame, San Bruno, South San Francisco and Menlo Park with flexibility to serve other ERF clients as needed. Key responsibilities include outreach and engagement, case management, participation in Multi-Disciplinary Team (MDT) meetings, and rapid response to community inquiries or referrals. The goal is to assist individuals in accessing housing and connecting to critical resources such as shelter, health services, and other support systems. **Case Management Duties:** - Provide intensive, field-based case management to unsheltered individuals, maintaining a caseload of 15–20 clients, with a minimum of twice-monthly in-person contact. - Develop and implement client-centered housing plans to assist individuals in rapidly transitioning from homelessness to permanent housing, following Housing First principles. - Conduct rapid response outreach and engagement within eight (8) business hours of the request. - Support collaboration through active participation in multidisciplinary team (MDT) meetings with key stakeholders, including law enforcement, San Mateo County BHRS, Street Medicine, homeless shelters, and other safety net providers, while ensuring compliance with confidentiality policies and proper consent protocols for information sharing. - Engage clients in trauma-informed, harm-reduction services, supporting access to shelters, health services, and housing resources without prerequisites such as sobriety or income. - Assist clients in obtaining documents, completing housing applications, and navigating referrals for services like substance use treatment, mental health care, and permanent supportive housing. - Collaborate with multidisciplinary teams, service providers, and housing programs to ensure warm handoffs and continuity of care when clients transition between programs. - Support clients post-housing transition with ongoing case management and resource connections for up to a year to promote stability and goal achievement. - Monitor and reassess client progress, coordination with community partners and maintaining consistent high-quality and up-to-date documentation and data. - Other duties as assigned by Supervisor. - Bachelor’s degree in social work, psychology or related field, with a minimum of two years’ case management experience.
