**Care Team Coordinator** The Care Team Coordinator serves as an administrative and regulatory coordination resource for the interdisciplinary team (IDT), ensuring compliance with PACE regulations and organizational standards. This role coordinates Service Determination Requests (SDRs), Plan of Care (POC) meetings, documentation, and required assessments while supporting efficient communication among the interdisciplinary team.
