As we continue to expand, we are currently seeking for motivated and organized Care Scheduler to join our leadership team and drive our managed care initiatives. **JOB DESCRIPTION SUMMARY:** Responsible for coordinating and scheduling care services for patients, ensuring the right caregivers are assigned to meet client needs. The Care Scheduler works closely with patients, caregivers, and healthcare teams to ensure seamless care delivery, maintain accurate scheduling, and manage patient and caregiver expectations. **ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:1. Coordinates and schedules care appointments for patients in accordance with their needs, ensuring proper caregiver assignment.** **2. Works closely with caregivers, patients, and families to confirm scheduling details and make adjustments as necessary to ensure continuity of care** **3. Communicates effectively with patients and families to understand their care needs and preferences, ensuring all scheduling details are clear and understood.** **4. Monitors caregiver availability and assigns appropriate caregivers based on skills, experience, and patient needs.** **5. Maintains accurate, up-to-date scheduling records for caregivers and patients, making adjustments as needed to ensure optimal care coverage.** **6. Ensures compliance with regulatory and company policies, including caregiver certifications, training, and patient care requirements.** **7. Responds to scheduling changes, cancellations, or emergencies in a timely and professional manner, ensuring minimal disruption to patient care.** **8. Collaborates with other departments to ensure timely and efficient scheduling of services, including coordinating with HR to ensure caregiver availability.** **9. Provides customer service to patients and families, addressing any concerns related to** **scheduling or caregiver availability.** **10. Maintains records of any patient or caregiver issues related to scheduling and communicates with management to resolve problems effectively.** **11. Assists with the recruitment and orientation of new caregivers, ensuring they are familiar with scheduling procedures.** **12. Regularly reviews and audits scheduling processes to ensure efficiency and accuracy.POSITION QUALIFICATIONS:1. High school diploma or equivalent required; Associate’s or Bachelor’s degree in a healthcare-related field preferred.** **2. At least two (2) years’ experiences in healthcare scheduling or a related field preferred** **3. Strong communication and organizational skills with attention to detail.** **4. Ability to manage multiple tasks and adapt to changes in a fast-paced environment.** **5.
