Care Manager

This role is ideal for a professional who understands the complexities of care transitions, discharge planning, and ongoing support to older adults and their families. The Care Manager is to oversee and coordinate care services for our clients in a home care setting. The Care Manager will serve as the primary point of contact for clients and their families, ensuring personalized, high-quality care that promotes independence and well-being. Additional territories include Sebastian, Port St Lucie and Fort Pierce. **Key Responsibilities (but not limited to):** Access clients’ needs through in-home evaluations and collaborate with families and healthcare providers.
