Mercy offers a NYS designated Stroke Center, Breast Imaging Center of Excellence, Level III Neonatal Intensive Care Unit, and has received Pathway to Excellence designation from the American Nurses Credentialing Center - the only hospital in the region and one of only two in NY State to receive this prestigious award. ## Job Details Responsibilities: - Perform discharge- planning assessment, when indicated, within the required timeframe. - Foster a collaborative relationship with the medical staff and other members of the health care team to provide quality patient care and reduce LOS. - Daily calls/ conversations to physicians regarding medical plan of care, documentation and Milliman Criteria and projected discharge date. - Calls/ conversations with physicians regarding discharge planning needs. Takes telephone orders, if necessary, to accomplish meeting the planning needs. (examples: order for pt/ ot, diet orders, discharge orders - Bedside rounding with physician as often as possible. - Collaborative with members of the health care team to achieve a cohesive and comprehensive discharge plan for patients. - Calling ancillary departments on a daily basis to facilitate the prioritization of testing and obtaining results. (Example: calling PT for early evals, calling dx cardio to have a test completed early with results in order to facilitate a discharge.) - Develop discharge plans that are responsive to patient/ family concerns, health care status, available resources, age-specific problems and diagnosis specific issues. - Develop discharge plans to ensure discharge and/or placement as soon as possible following resolution of acute care treatment. - Setting up discharge plan based on the assessment- dual plan if necessary. Finalize plan prior to end of acute illness and setting up day of potential discharge. This will require taking the lead and informing team/ MD/ MLP of time of discharge/ pick-up. - Provide appropriate referrals for patient/family to various resources. - Document all discharge planning activities in a timely and accurate manner. - Ensure the medical necessity of all new admissions and continued stays. - Take appropriate action to resolve operational bottlenecks in hospital services. - Working directly off of the daily priority list generated from rounds. Strong follow up with all members of the team to ensure that all work is completed timely in an effort to prevent sluggish management, improve quality of care and reduce los. (Example: making sure that the proper hepatitis panel is performed on dialysis patients, making sure discharge paperwork is completed by the team, follow up with MLP or PMD with specific information that was needed for decision making. - Facilitate the completion and/or completes as appropriate, all required forms in an accurate and timely manner. (E.g. Screens, Transfer forms, PRI’s, HINNS, Generic Screens, & Q.A. & P.I. forms as required, etc.) - Complete departmental forms as required, e.g. ALC Log, Insurance review Log, Change in Level of Care Form. - Refer variations to the P.A./Director of Service and/or other departments as appropriate. - Coordinate and attend daily multidisciplinary rounds on assigned unit. - Perform focus studies and screening activities in accordance with established occurrence criterion. - Perform third party payer reviews as requested within the appropriate timeframe and document it. - Submit monthly statistics by the first working day of the new month.
