Care Manager HRSN

**Cayuga Counseling Services** is seeking a **Care Manager** to provide Care Management for eligible Medicaid members under the Medicaid 1115 wavier program. The **Care Manager** is responsible for the following: - Provide Care Management services that address the health-related social needs of eligible members under the Medicaid 1115 waiver which can include housing, nutrition and transportation needs. - Assist eligible members with securing safe, affordable housing. - Follow up with members to ensure they receive the services they were referred to. - Refer eligible members to programs within the Network. - Maintain strict compliance with HIPPA and all federal, state and agency confidentiality regulations, policies and procedures. - Document all services and outreach per program policies and procedures. - Attend various meetings to educate providers about the services available under the wavier. - Attend all required trainings. - Performs other duties as needed. **Knowledge, Skills, and Abilities:** - Experience with working with Medicaid or vulnerable populations preferred. - Strong computer skills, including experience with Microsoft Office and electronic documentation systems. - Knowledge of social services and community resources. - Strong documentation and organizational skills - The ability to work independently. **The Agency offers:** - **3** weeks of paid time off in your first year, **4** weeks in your second year. - **48** hours of **Wellness time** annually (4 hours accrue per month) that can be used any time you feel you need some time for yourself. - **96** hours of **Sick time** annually (accruing 8 hours per month) - **10 Holidays** off per year including **One Floating Holiday** - Agency sponsored Life and Dental insurance for employees, family plans are also available. - **6** different **health insurance plans** to choose from to fit your needs. - Supportive work environment with an opportunity to grow and receive agency sponsored trainings. - **Sabbatical**: After 5 years of service, you are eligible for **4 WEEKS PAID TIME OFF** that does not go against your other PTO. - **EEO**
