Care Coordinator II - Delegated

**JOB CATEGORY: CARE COORDINATION / CASE MANAGEMENT / SOCIAL WORK / REGISTERED NURSE** **POSITION SUMMARY:** Under licensed supervision, the Care Coordinator II – Delegated works closely with the patient/family and the Interdisciplinary Care Team to proactively assess, plan, implement, monitor and evaluate options and services to effect an appropriate, individualized plan for the patient across the continuum of care. The process addresses the patient’s multiple environments of care and healthcare system needs. The position requires a focus on customer service skills, knowledge of goal setting, and measuring outcomes to effectively ensure the patient has the most optimal outcomes provided in the most cost-effective manner. Adheres to procedures and timelines as specified in the Managed Care Organization (MCO) contract and PMS procedures. ### Qualifications **REQUIREMENTS** **EDUCATION AND/OR EXPERIENCE:** - Associate’s degree AND five year’s relevant healthcare experience; OR - Bachelor’s degree AND four year’s relevant healthcare experience; OR - Master’s degree AND two year’s relevant healthcare experience. - Good knowledge of personal computer and software for word processing and the ability to retrieve data from relevant computer systems. - Self-starter with strong verbal and written communication skills. - Highly organized with excellent follow-through skills. - Comfort with change and quality improvement processes. - Required to pursue ongoing education, certification, and/or self-development to remain current with industry standards and business objectives related to care coordination. **Preferred** - Experience with Electronic Health Records. - Experience with Motivational Interviewing. - Knowledge of New Mexico and local community resources. - Bilingual (English / Spanish) or (English / Navajo). **CERTIFICATES, LICENSES, REGISTRATIONS:** - Certified Case Manager (CCM) preferred. - Have or be able to obtain First Aid and BLS/CPR certifications within 30 days of hire. Certifications must meet the requirements listed in the document “BLS / CPR / ACLS & First Aid Training Resources” posted on Staurolite. It is your responsibility to keep your certifications current at all times. **SCREENING REQUIREMENTS:** This position requires successful completion of an initial post-offer of employment Criminal Records Check. - According to PMS Human Resources Policy 205.006, “Healthcare Personnel Testing & Immunization Requirements,” this position must comply with required testing and immunizations. Testing and immunizations including TB, Hepatitis B (consent or waiver), Tdap, MMR, and Varicella, will be available at the employee’s PMS health center.
