### **Now Hiring: Care Coordinator – Children’s Long-Term Support (CLTS) Program** *Walworth County, WI* *Full-Time | M–F, First Shift | Hybrid Work Option for **Wisconsin** residents.* *Make a difference in the lives of children and families!* **Lutheran Social Services of Wisconsin & Upper Michigan** is seeking a dedicated **Care Coordinator** to join our **CLTS Waiver Program** team in **Walworth County**! The **Children’s Long-Term Support (CLTS) Waiver Program** is a federally funded Medicaid initiative that helps children with developmental, physical, or severe emotional disabilities receive services that allow them to live safely in their homes and communities.
