Care Coordinator - Behavioral Health

Mental Health Kokua (MHK) is hiring a full-time **Care Coordinator** to support residents living in the Kauhale supportive housing community at the Iwilei Center on **Oahu**. This role focuses on helping residents maintain stability by coordinating health services, appointments, and essential resources that support recovery and independent living. In this role, you will work closely with residents, case managers, and community providers to ensure individuals have consistent access to care, transportation, benefits, and wellness supports.
