## Description The **REGISTERED LONG-TERM CARE NURSING ASSISTANT** **(RLTCNA)** is a member of the Nursing Department and health care team and is responsible for the direct provision of care and services to the residents in a manner that is consistent with the facility mission and principles that assures the highest practical level of quality care and services to our residents and other customers. Essential Job Duties**:** The Registered Long Term Care Nursing Assistant: Performs assignments for resident care and other related duties as received from the licensed nurse on the unit to which assigned. Provides direct personal care services to each assigned resident including but not limited to: Assisting each resident in performance of activities of daily living: grooming, bathing, mobility, eating and toileting. Makes occupied and unoccupied beds. Assures that each resident has bathing and grooming supplies as needed. Assures that clothing articles and supplies are used ONLY for the resident to whom they belong. Articles without names must be appropriately labeled for the owner. Personal care supplies stored in a manner which is in accordance with established safety and infection control practices -- separate from cleaning products, properly labeled for individual use, clean and free of hair or other evidence of soiling. Cleans nursing-related equipment and other work areas such as the nourishment center and utility rooms as assigned and properly stores supplies. Assures the availability of fresh water at the bedside at all times and regularly offers it to the resident. Assists in maintaining the cleanliness and orderliness of the resident’s living space in a manner that supports infection control practices. Assists residents to the Dining Room and Activity Programs as scheduled. Assists in labeling personal clothing and belongings as needed. Assists with the prompt distribution, set-up (opening cartons, eating utensils, containers, etc.) of meal trays and supplemental feedings to assure safe food distribution and temperature control. Provides assistance with feeding of residents as indicated based upon the individual plan of care. Assures that daily care includes provision of prompting and/or assistance with restorative activities related to the plan of care addressing the needs of the resident. Answers call lights promptly and reports care needs to the supervising nurse as appropriate. Monitors and assists residents as necessary to meet the toileting needs. Protects the resident’s privacy and dignity while performing personal care. Uses a gait belt for all transfers. Assures that all contacts with the resident, family and other customers are treated with respect and courtesy by: Knocking before entering a room; Assists the resident to maintain personal hygiene and appearance at all times; Treats personal belongings with care; and maintains respectful control of environmental sounds and lighting levels maximizing reality orientation and minimizing confusion and distractions. Assures that necessary steps are taken for the prevention and early identification of life-threatening conditions and quality concerns such as dehydration, fecal impaction, and pressure sores. Attends meetings and participates in educational programs as scheduled and completes all mandatory requirements for training within allowable timeframes. Reports off to the nurse whenever leaving the unit or going off duty. Maintains strict confidentiality of all residents and facility information to which exposed. Follows facility rules for personal safety and for the safety of each resident, visitor and other members of the staff. Participates in the care planning process for assigned residents. Works with other members/departments on the facility team as part of the Quality Assessment and Assurance process in seeking to identify ways to improve on the care and services to the resident population.
