POSITION OVERVIEW Provides housing-focused case management and community engagement services for families and Spanish-speaking households experiencing or at risk of homelessness. Delivers Housing Problem Solving, Homelessness Prevention, Coordinated Entry, outreach, employment navigation, workshops, and referrals in accordance with Alameda County Housing Resource Center standards and the Abode Services subcontract. This role will uphold and champion the agency’s mission, vision, and values and contribute to a collaborative and inclusive work culture. This position is covered under the SEIU 1021 Collective Bargaining Agreement. In addition to the established base pay, members will receive negotiated annual increases as follows: - January 2027: 3% base pay increase Future increases will be subject to the terms of the collective bargaining agreement. ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES: - Conduct Housing Problem Solving conversations and develop individualized housing stabilization plans. - Complete Coordinated Entry access, crisis, housing, and prevention assessments. - Provide homelessness prevention services and coordinate financial assistance requests. - Conduct community outreach and engage households in shelters, community locations, and partner sites. - Maintain accurate, timely HMIS documentation and data quality. - Provide ongoing case management, monthly client engagement, and Current Living Situation updates. - Facilitate workshops on housing, tenant education, employment, benefits, healthcare access, and housing stability. - Provide light-touch employment navigation including resume assistance, interview preparation, and referrals. - Coordinate referrals to healthcare, behavioral health, benefits, legal, workforce, and community resources. - Collaborate with Prevention HUB partners, Coordinated Entry, outreach teams, and community providers. - Participate in required trainings, learning communities, and quality improvement activities. - Prepare reports and maintain compliance with HUD, HIPAA, HMIS, County, and agency requirements. - Provide culturally responsive, trauma-informed, Housing First, strengths-based services. - Perform other duties as assigned.
