Become part of our team and make a meaningful impact in the community every day! ## Description **Make a difference every day at Amoskeag Health, where we believe strong healthcare begins with strong human connections!** **Who You Are:** Amoskeag Health is seeking a compassionate **Behavioral Health Clinician** to support children, adolescents, adults, and families within an integrated care setting. You are committed to delivering trauma-informed, culturally responsive care and thrive in a collaborative, fast-paced environment. You bring strong assessment skills, value teamwork, and are dedicated to improving both individual and population health outcomes. **What You'll Do:** - Work effectively as part of a multi-disciplinary team, collaborating to develop treatment plans and to co-manage care of patients. - Provide comprehensive assessment, diagnosis, and treatment interventions to patients served in an integrated primary care health clinic. - Utilize standardized, evidence-based clinical measures to assess patient’s symptoms and response to treatment, through individual and group therapy services. - Provide effective treatment planning and assist patients to set and work toward goals to improve physical and psychosocial functioning. - Provide crisis intervention, crisis management, and safety planning services as appropriate. - Provide patients with linkages to community services, resources and supports. - Provide consultation to medical providers and other team members as needed. - Contribute to data collection and reports as requested by supervisor. - Manage a caseload size adequate to generate 62.5% in billable services per week (25 clinical hours/week). - Document patient progress and treatment recommendations in Electronic Medical Record and other required systems to be shared with medical providers and other interdisciplinary team members. - Submit billing, clinical, and other related documentation within five days of the provided service. - Actively participate in meetings that support Amoskeag Health’s integrated model of care. - Assess and assist patients in need for referrals, specialty mental health or substance use services, outside of primary care, to connect with appropriate treatment referrals. ## Requirements **What You'll Bring:** - Master's Degree or higher required in Social Work, Psychology, Mental Health Counseling or Marriage and Family Therapy - Working towards State of New Hampshire License. Independently Licensed preferred (LICSW, LMHC/LMFT, MLADC) - Two years’ experience in the healthcare field preferred - BLS certification required - Verifiable good driving record and reliable transportation - Fluent in English reading, writing, and understanding is required for patient safety. Spanish is preferred but not required
