## Overview • Supports a team of professionals working collaboratively to assist youth and adults with intensive behavioral needs. • Completes discharge or linkage to other additional services as needed. • Conducts a thorough diagnostic assessment using interviews, direct observation of participants, reviewing individuals’ history, testing, and data. • Collaborates with the individual’s team to develop and implement individualized treatment plans through individual and group work, monitors, and adjusts goals, collects and analyzes data, and completes necessary updates as required. • Communicates with families, service providers, probation officers, case workers, schools, and other interested parties to exchange necessary information during the treatment process and documents all participant-related contact in compliance with agency standards. • Provides oversight and supervision to assigned staff by monitoring work duties and responsibilities, establishing and modifying work methods, schedules, and priorities. Monitors TCM Billing. Elevates critical issues to manager. • Develops and maintains positive working relationships to provide information and education on the role of Summit DD’s behavioral support resources to team members. • Connects with service providers and families to identify gaps impacting implementation of recommended strategies and acquires available information to determine effectiveness of treatment. • Works with provider staff and families to train and implement behavior support strategies. • Provides crisis intervention services and is available for consultation. • Facilitates referrals to appropriate internal and/or external resources as needed. • Participates in agency and community committees and meetings as needed. • Identifies needed training opportunities and collaborates with appropriate staff to coordinate training. • Maintains all required service documentation. ## Description This position is responsible for completing assessments, developing treatment plans, designing support strategies, and assisting with care coordination for individuals eligible for services. Provides supervision and is a member of an interdisciplinary team. Works collaboratively with the provider community. ## Requirements Master’s degree in social work, counseling, or closely related behavioral science field. The degree must be from an accrediting entity (i.e., CSWE/ Council on Social Work Education, CACREP/ Council for Accreditation of Counseling and Related Education Programs). One year post licensure experience as an LISW, LPCC, or through a similar individual license. • Maintain active independent licensure from the Ohio Counselor, Social Worker, and Marriage and Family Therapist Board (such as LISW-S or LPCC-S). • Obtain and maintain Service & Support Administration Supervisor Certification • Previous Mental Health experience in social work or counseling. • Strong documentation skills. • Motivated, organized, and detailed. • Knowledge of community resources is helpful. • Ability to build rapport with clients and direct support professionals. • Strong leadership qualities. • Strong facilitation skills. • Good problem-solving skills. • Excellent written and verbal communication skills. • Good computer skills. • Ability to work as part of a team and collaborate effectively with people and communities from diverse cultural, social, and ethnic backgrounds. • Report unusual incidents and major unusual incidents. • Knowledge or experience of the Developmental Disabilities system a plus.
