Responsibilities: As a Band 6 Physiotherapist, you will be responsible for facilitating discharge planning and coordinating the movement of patients from acute wards to onward services, ensuring continuity of care. This involves assessing patients, identifying discharge needs, and working closely with multidisciplinary teams, including nursing staff, occupational therapists, social workers, and medical teams. You will manage complex cases, ensuring that patients are safely discharged with appropriate support in place. The role requires flexibility to cover the 7-day service, including shifts over weekends and public holidays. You will be responsible for maintaining accurate records, liaising with external healthcare providers, and ensuring that patients receive the best possible care as they transition out of hospital. Qualifications and Experience: HCPC registration as a Physiotherapist Degree in Physiotherapy or equivalent Extensive discharge experience within an acute hospital setting Experience working as a Senior Physiotherapist, ideally with a focus on discharge coordination Ability to work in a multi-disciplinary team and handle complex discharge planning Strong communication skills and ability to work under pressure Ability to contribute to a 7-day service, including public holidays Enhanced DBS check required Previous experience working within the NHS is highly desirable.
