This position requires a strong background in intermediate care and rehabilitation, with a focus on frail and elderly patients, and experience in a community setting. What your responsibilities will be for this Physiotherapist role? As a Band 6/7 Locum Community Physiotherapist, you will provide comprehensive physiotherapy services within the community setting of the Jean Hayes Reablement Unit. Your responsibilities will include conducting thorough assessments of patients, developing and implementing personalised rehabilitation plans, and delivering interventions to support their recovery and improve functional independence. You will manage a caseload that primarily includes frail and elderly patients, ensuring high-quality care and effective management of their needs. Your role will involve working collaboratively with other healthcare professionals to deliver integrated care, and you will be expected to maintain accurate patient records and contribute to the development of the rehabilitation service.
