This role focuses on ward-based medicines management activities, supporting safe and effective patient care through accurate medicines reconciliation, patient counselling, and clinical support. The successful candidate will play a key role in ensuring medicines are used safely and efficiently across inpatient wards, working closely with pharmacists and multidisciplinary teams. Key Responsibilities Undertake medicines reconciliations for patients on hospital wards. Perform accuracy checking duties where appropriate. Conduct controlled drug (CD) audits and support compliance. Provide patient counselling on medicines and support safe discharge processes. Work collaboratively with pharmacists and ward teams to optimise patient care. Qualifications and Experience Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC). Proven experience undertaking medicines reconciliation in a hospital setting (essential). ACPT qualification desirable but not essential. Previous NHS hospital experience preferred.
