The work schedule for this position is aligned with Erie's hours of operations. **Main Duties & Responsibilities** - Provide one-on-one health education and self-management support to patients - Participate in community events, training, and AmeriCorps service activities - Support Lending Hands for Life HIV programming and re-engagement efforts - Conduct outreach and engagement for patients requiring additional support - Document patient interactions and coordinate follow-up care activities - Facilitate health education groups and recruit participants for programs \*\*Applications must be submitted by **September 13, 2026,** to be considered for full-time opportunities.
